Hemobilia and Mirizzi Syndrome: A Rare Combination

چکیده
A 64-year-old obese woman presented with sudden onset of abdominal pain and hematemesis. Physical exam was notable for tenderness in the right upper quadrant (RUQ) with a positive Murphy’s sign. Lab results showed elevated aspartate aminotransferase 291 U/L, alanine aminotransferase 231 U/L, and total bilirubin 2.3 mg/L. A RUQ ultrasound showed cholelithiasis. Total bilirubin increased to 4.3 mg/L in the next 24 hours. Endoscopic retrograde cholangiopancreatography (ERCP) revealed a large filling defect in the proximal common bile duct (CBD), close to the takeoff of the cystic duct and indicative of Mirizzi syndrome, and blood clots extruding through the major papilla consistent with hemobilia (Figure 1 and Figure 2). Balloon catheter removal of the stones was unsuccessful due to the largest stone appearing to be within the gallbladder fossa; a biliary stent was placed for biliary drainage. Computed tomography showed a large gallstone with

Hemobilia due to arteriobiliary fistula complicating ERCP for residual bile duct stone in a case of Mirizzi syndrome
Hemobilia is a rare cause of upper gastrointestinal tract bleeding. Most cases are iatrogenic following medical interventions, most commonly liver biopsy and transhepatic cholangiography. We present a case of arteriobiliary fistula between the right hepatic artery and the common hepatic duct, in a case of Mirrizi syndrome, following endoscopic biliary stenting and presenting with hemobilia. The...

Mirizzi Syndrome is a rare and challenging clinical entity to manage. However, recent advances in technology have provided surgeons with new options for more effective diagnosis and treatment of this condition. This paper reviews these new diagnostic modalities and treatment approaches for the management of Mirizzi Syndrome. An online search language was performed using PubMed and Web of Science...
متن کاملMirizzi Syndrome: From Ultrasound Diagnosis to Surgery—A Case Report
The Mirizzi syndrome is a rare disorder that usually presents with jaundice and cholangitis; its lack of recognition in the diagnostic path could have serious consequences for the patient undergoing cholecystectomy. Here we describe the clinical case of a jaundiced patient from the ultrasound suspect of Mirizzi syndrome to the surgical treatment.
متن کاملMirizzi syndrome type IV associated with cholecystocolic fistula: a very rare condition- report of a case
BACKGROUND Mirizzi syndrome is a rare complication of prolonged cholelithiasis with presence of large, impacted gallstone into the Hartman's pouch, causing chronic extrinsic compression of common bile duct (CBD). Fistula formation between the CBD and the gallbladder may represent an outcome of that condition. According to Mirizzi's classification and Csendes's subclassification, Mirizzi syndrom...
متن کاملMinimally Invasive Treatment of Mirizzi Syndrome, a Rare Cause of Cholestasis in Childhood
Mirizzi syndrome is the compressive blockage of the cystic or choledochal duct caused by a biliary stone occupying the cystic canal or Hartmann's pouch. This occurrence is rare and, in English literature, three cases defined in children have been observed. In order to draw attention to this rare occurrence, we preferred a 14-year-old male patient with Mirizzi syndrome. In this case, ERCP was pe...
